Lavan Dementia Care Services is a specialist nurse‑led community service based in Guisborough, Redcar & Cleveland.
Established in January 2024, the organisation was created to continue providing high-quality dementia care that began under the region’s Specialist Admiral Nurse programme.
It supports individuals and carers affected by dementia through a range of flexible, dignified services, accepting both private and adult social care funding.
The core offering is the dementia care day service hosted at Sleights Community Hub.
These nurse-led sessions run five days a week and accommodate up to ten service users at a time, with two emergency places reserved daily for those in urgent need.
Lavan Dementia Care Services hosts a broader community hub at Sleights, welcoming carers, older adults, and people living with dementia.
The hub offers drop-in wellbeing groups, creative activities, gentle exercise, peer support, and occasional themed events—for example pony-therapy sessions and regular memory cafes.
All activities are free to access and designed to reduce isolation, increase resilience, and strengthen social connection.
